I scanned my HD with DriveDX and it revealed a "199 UDMA CRC Error Count". There's also an error message saying:

"Total count of errors in data transfer via the interface cable. Each time the error is detected, controller requests a retransmission, thus slowing down the overall transfer speed. Increase of raw value could indicate problem with the power supply or data cable."

Is there anything I should do? Here is the full system report. I've also attached the drive health indicator report:

With 52.7% Overall Health Rating I would be very concerned the drive is on the way out, together with errors listed in the report. Make sure you always have a current backup.

What to do about it? Boot into Recovery, go to Utilities and run Repair Disk and see what is rep[orted. One could always purchase utilities such as DiskWarrior, but at $120 think the money is better spent towards an SSD hard drive. An SSD will give your 2011 MBP a tremendous boost and for mine I would go with an OWC, Crucial or OCZ drive of 512GB.

Clone it first, it will make transition easy.

How long before it completely dies? Should I replace the data cable as well?

That's a "how long is a piece of string?" question. There is no way to predict how long any hard drive will last before it dies irrevocably. Suffice to say that you (unlike some) have an indicator which suggests that it may be soon. Many people have no warning at all. As for the data cable, it should be fine but you will never have a better opportunity to replace it. Others may have an opinion on that.

Anecdotally, my wife replaced her 500Gb HDD in her 2011 MBP with a 500Gb SSD and it has improved performance markedly despite only having 4Gb RAM running macOS Sierra.
