Friend was on FaceTime when the background of the person she was speaking with changed to fireworks with a thumbs up emoji. They ended the call, but this sounds like a hack to me. Is that even possible? Quick search in Apple support community reveals that it has happened to others. My friend is quite challenged with any tech, so if there is a solution is would have to be very, very simple. TIA

Not a hack, that user has Reactions enabled. So when you do specific things with your fingers like giving a thumbs up, thumbs down, heart and so on, you get those graphical things.

Read this article and the Reactions section.

Thank you much - problem solved!

There are a whole range of standard reactions, thumbs up, thumbs down, balloons, confetti, fireworks, love hearts and rain storm, all dependent on "gestures". Very popular for video conferencing eg. Zoom.
